In general terms, transition is defined as “a process or a period of changing from one state or condition to another”.
For IBD patients the term transition is often used to describe the process of movement and change from being treated under the paediatric system to being classed as an adult patient and starting treatment in the mainstream IBD system as an adult. Over the past number of years IBD patients are being diagnosed at a much younger age, with a large increase in children being diagnosed. On the back of this surge in children being diagnosed, it will lead to a greater volume of children transitioning from paediatric care to mainstream adult care.
